The Clinic Plan Program provides you with unlimited access to numerous routine outpatient services at three International SOS clinics in Indonesia and a defined list of free medicines for one prepaid fee, per person, per year. The type of Clinic Plan in Indonesia:

The Clinic Plan is not medical insurance. It is a routine prepaid medical plan where services are offered only on an out-patient basis in the Clinics.
Please note that while your Clinic Plan program entitles you to a broad range of free medical services and prescriptions, some procedures and prescriptions fall outside the program and are chargeable.

Instructions on how to read your Clinic Plan program card are provided below.
RED CARD (FRONT)

| 3. | Your clinic file number is detailed at the barcode on the back of Clinic card. This is to assist you to make an appointment when visiting the clinic. |
How to Use Your “Clinic Plan” Card - International SOS Clinic phone numbers are detailed on the reverse of your "Clinic Plan" membership card. This is to assist you in contacting the clinic to make an appointment prior to visiting the clinic.
- Your personal details are specified on the front of the card for recognition purposes and easy access to your records.
- Your clinic file number is detailed on the reverse of the card. This number will be scanned whenever you visit the clinic to see a doctor, in order to access your file easily and reduce time taken to see a doctor
- Simply present your Red Clinic Plan card at reception on arrival at one of our clinics.
- Please note that charges will apply for any procedures/medications not covered by the program.
How to Renew Your “Clinic Plan” Card - International SOS staff will make contact with you or your company two months prior to the expiry of your pre-paid program and again one month prior to expiry of your current membership. Program benefits will not be available if the card has expired.
- Our membership staff will also advise you in writing and attach the relevant renewal forms for your completion and return to International SOS.
- Upon completion of the forms, you may either hand deliver them to our offices or fax them to (021)765 5950.
- Payment of the renewal amount needs to be made prior to activating your renewed membership. Such payment needs to be made at our offices or can be done electronically to the bank account specified on the renewal forms.
- Once your renewal has been processed, International SOS will send you your new "Clinic Plan" membership card which has a barcode with your clinic file number on the reverse. The old card may then be disposed of.
Procedures and Drugs Covered Under “Clinic Plan” Listed below are the range of medical services and medications covered under the Clinic Plan program.
Prepaid Clinic Plan Services
- Unlimited consultations with General Practitioners
- Consultations with Specialists by Appointment (Psychiatrist, Psychologist, Acupuncturist, Nutritionist and special appointments are not covered)
- Routine childhood vaccinations and boosters (see Medications covered by Clinic Plan)
- Hepatitis B and Typhoid Vaccinations
- Routine medications and prescriptions (see Medications covered by Clinic Plan)
- Should a specific drug not be available then a suitable generic will be supplied if available
- Drugs will be prescribed based on the diagnosis made during the current consultation
- Drugs will be prescribed only for the customary number of days required for the illness
- Routine laboratory tests (as medically indicated)
- X-rays (non-contrast) (as medically indicated)
- Ultrasonography (USG) (as medically indicated)
- Electrocardiograms (ECG) (as medically indicated)
- One Dental Check Up and Scale & Polish per year (by appointment)
- Selected dressings and disposables
- Observation in the Emergency Room – 2 hours
- International Standard Ambulance (Jakarta area/Airport) – 2 hours (for Hospital transport in the Jakarta area or to the Airport)

- Subscriptions are transferable within the company, but Clinic Plan cards must be in the name of the user.
- Additional subscriptions may be added on a pro-rata basis for companies, with a minimum extension of 6 months.

Clinic Plan – Terms and Conditions
- The program must be paid annually or 6 months in advance prior to accessing the benefits.
- Benefits and Services can only be used at International SOS Clinics in Indonesia. For details of these clinics refer to International SOS Clinics in Indonesia.
- One program needs to be purchased for each member.
- Benefits can only be used as per the details listed in Procedures and Drugs Covered under Clinic Plan.
- Drugs will be prescribed based on the diagnosis made during the current consultation.
- Drugs will be prescribed only for the customary number of days required for the illness.
Frequently Asked Questions How do I join the "Clinic Plan" plan?
All you have to do is complete an application form detailing contact numbers, participants in your family or company and forward payment to our bank, details of which are listed on the application form.
How do I use Clinic Plan?
Once the application has been processed and payment received, you will receive a Clinic Plan card, which will be valid in two primary clinics at Jakarta. Please show your card whenever you use the service and remember to make an appointment prior to your visit.
How do I know what is covered and what is not?
