    This card is unique to SOS Medika public clinics in Indonesia and is not intended for use outside of Indonesia. Please ask for details of International SOS Clinic services if relocating to another country.

    This patient file number is unique to you. It indicates where your medical file was initially opened with International SOS.
    2. Clinic addresses and contact numbers for International SOS clinics in Indonesia are detailed here. Please contact the nearest Clinic when needing to see a doctor, dentist or specialist.
    3. This Website address provides you with details of all the features and benefits of your Clinic Gold Card. Details of all International SOS Clinic services in Indonesia are provided here too.


    How to Use Your Gold Clinic Card Top

    1. YOUR FIRST VISIT:
    The fist time you come to an SOS Medika Clinic to see a physician, we will ask you to complete the patient registration / history form, so that we may set up a patient file for you. If you have a copy of your medical records from your previous doctor, we will glad to add these to your new SOS Medika file. You will also be supplied with a Gold Clinic Card. This card has to be produced every time you visit the clinic facility. Your clinic file number will be put onto this Gold Clinic Card.

    2. APPOINTMENTS:
    We encourage you to call for an appointment for all of your visits to the clinics. Having an appointment will allow us to serve you better and minimize your waiting time. Some of our specialists practice only at certain time on certain days and hence it would be necessary to make an appointment to see any of our specialists.

    3. CLINIC PROCEDURES:

    • Proceed to the Reception Desk.
    • Show your Gold Clinic Card
    • Request to see a doctor.
    • After seeing the doctor, you may be required to have lab test, x-ray or fill a prescription.
    • Report to the cashier to pay for your treatment and service.

    4. HOME VISIT:
    If you are not able to come to the clinic due to illness, you can request for an International SOS doctor to do a home visit. This is an expensive service and should be used only when absolutely necessary. International SOS should be advised of the nature of the problem by phone so that the doctor can bring any special equipment or drugs that may be needed.

    5. COORDINATOR DOCTORS:
    If you are an International SOS member and want to speak to a Coordinator Doctor (CD), please call International SOS Alarm Center on (62) (21) 750-6001. The staff will ask your name, company, membership number and phone number where we can reach you. Our staff may need to locate your file for the Coordinator Doctor who will return your call.

    6. FOR MEMBERS SEEKING A SECOND OPINION AT THE SOS MEDIKA CLINIC:
    If you are requiring a second opinion from our CD, you can request that through our reception desk or the Nurse in the Areas (only available at SOS Medika Clinic, Cipete).

    7. TREATMENT OUTSIDE SOS MEDIKA CLINICS:
    If you need medical treatment in an area where International SOS does not have a clinic, please all our Alarm Center on (62) (21) 750-6001. Our CDs will advise you on the best specialist in the area to handle your specific needs. Our doctors can also give you advise on any lab tests and drugs that your local doctor may be prescribing.
